కోవిడ్-19 నిర్ధారణ పరీక్షలు సార్స్-సీవోవీ-2 వైరస్ను గుర్తించగలదు. అంతే కాకుండా ఇది వైరస్ ఉనికిని గుర్తించే పద్ధతులు(ఆర్టి-పిసిఆర్, ఇసోథెర్మల్ న్యుక్లిక్ ఆసిడ్ ఆమ్ప్లిఫికేషన్), సంక్రమణకు ప్రతిస్పందనగా ఉత్పత్తి అయ్యే యాంటీబోడీస్ గుర్తించే పద్ధతులను కలిగి ఉంటుంది. రోగనిర్ధారణ, జనాభా పర్యవేక్షణ కోసం ప్రతిరోధకాలను గుర్తించే ప్రక్రియని (సీయెరాలజీ) ఉపయోగించవచ్చు. యాంటీబోడీస్ పరీక్షలు ఎంత మంది ఈ వ్యాధిని కలిగి ఉన్నారో, వీరిలో ఎంత మందికి వ్యాధి లక్షణాలు ఉన్నాయో లేక అసలు లక్షణాలు లేవో కూడా చూపిస్తాయి. ఈ పరీక్ష ఫలితాల నుండి వ్యాధి ఖచ్చితమైన మరణాల రేటు, జనాభాలో రోగనిరోధక శక్తి స్థాయిని నిర్ణయించవచ్చు.

పరిమిత పరీక్షల కారణంగా, మార్చి 2020 నాటికి ఏ దేశమూ వారి జనాభాలో వైరస్ ప్రాబల్యంపై నమ్మదగిన డేటాను చుపించాలేకపోయాయి.[1] ఏప్రిల్ 29 నాటికి, వారి పరీక్ష డేటాను ప్రచురించిన దేశాలు సగటున వారి జనాభాలో 1.4% కు సమానమైన పరీక్షలను నిర్వహించాయి. ఏ దేశమూ దాని జనాభాలో 14% కంటే ఎక్కువ నమూనాలను పరీక్షించలేదు.[2] వివిధ దేశాల పరీక్షల సంఖ్యలలో తేడాలు ఉన్నాయి.[3]

పరీక్షా పద్ధతులు[మార్చు]

ఆర్టి-పిసిఆర్(RT-PCR)[మార్చు]

అకృత్రిమ సమయ రివర్స్ ట్రాన్స్క్రిప్షన్ పొలిమేరాస్ చైన్ రియాక్షన్ (RT-PCR) వాడి, నాసొఫారింజియల్ శ్వాబ్ లేదా స్పుటుం సమూనా సహా వివిధ పద్ధతుల ద్వారా పొందిన శ్వాసకోశ నమూనాలపై పరీక్ష చేయవచ్చు.[4] ఫలితాలు సాధారణంగా కొన్ని గంటలు నుండి 2 రోజుల వ్యవధిలో లభిస్తాయి.[5] త్రోట్ శ్వాబ్స్ తో చేసిన ఈ పరీక్ష వలన వచ్చిన ఫలితాలు, వ్యాధి మొదటి వారంలో మాత్రమే నమ్మదగినవి. తరువాత ఈ వైరస్ గొంతులో అదృశ్యమవుతా ఊపిరితిథుల్లో గుణించడం కొనసాగిస్తోంది. రెండవ వారంలో పరీక్షించిన సోకిన వ్యక్తుల కోసం, ప్రత్యామ్నాయంగా మాదిరి పదార్థాన్ని లోతైన వాయుమార్గాల నుండి సక్షన్ కాథెటర్ ద్వారా తీసుకోవచ్చు లేదా కఫం ఉపయోగించవచ్చు.

ఐసోథెర్మల్ ఆంప్లిఫికేషన్ అస్సేయ్స్[మార్చు]

27 మార్చి 2020న ఐసోథర్మల్ న్యూక్లియిక్ యాసిడ్ యాంప్లిఫికేషన్ పద్ధతిని ఉపయోగించే అబోట్ డయాగ్నోస్టిక్స్ నుండి "ఆటోమేటెడ్ అస్సేస్" ను ఎఫ్డిఎ(FDA) ఆమోదించింది.[6]

సీయెరాలజీ[మార్చు]

చాలా సీయెరాలజీ పరీక్షలు అభివృద్ధి పరిశోధన దశలో ఉన్నాయి.[7] ఏప్రిల్ 15 నాటికి, యునైటెడ్ స్టేట్స్లో ఎమర్జెన్సీ యూస్ ఆథొరైసేషన్ (EUA) కింద రోగ నిర్ధారణ కోసం నాలుగు పరీక్షలు ఆమోదించబడ్డాయి. ఈ పరీక్షలు కెంబయో డయాగ్నోస్టిక్ సిస్టమ్, ఆర్థో క్లినికల్ డయాగ్నోస్టిక్స్, మౌంట్ సినాయ్ లాబొరేటరీ, సెల్లెక్స్ ద్వారా చేయబడతాయి. నాలుగు పరీక్షలు తప్పనిసరిగా ప్రయోగశాలలో చేయాలి.[8][9][10] సెల్లెక్స్, కెంబయో చేసిన పరీక్షలు రాపిడ్ డయాగ్నొస్టిక్ టెస్ట్స్ (RDT) ఫలితాలను ఇవ్వడానికి 10-30 నిమిషాలు పడుతుంది. ఆర్థో, మౌంట్ సినాయ్ చేసిన పరీక్షలు ఎంజైమ్ లింక్డ్ ఇమ్యునోసోర్బెంట్ అస్సే (ELISA) పరీక్షలు, ఇవి ఫలితాలను ఇవ్వడానికి 1–5 గంటలు పడుతుంది. చైనాలో, సెల్లెక్స్ పరీక్ష 95.6% నిర్దిష్టతను, 93.8% సున్నితత్వం కలిగి ఉంది. ఇతర దేశాలలో వేరువేరు పరీక్షలు ఆమోదించబడ్డాయి.[7]

ఈ పరీక్షలను ఉపయోగించి అనేక దేశాలు తమ జనాభాపై పెద్ద ఎత్తున సర్వేలు ప్రారంభిస్తున్నాయి. కాలిఫోర్నియాలో ఒక అధ్యయనం ఒక కౌంటీలో యాంటీబాడీ పరీక్షను నిర్వహించి కరోనావైరస్ కేసుల సంఖ్య జనాభాలో 2.5, 4.2% మధ్య ఉందని లేదా ధృవీకరించబడిన కేసుల సంఖ్య కంటే 50 నుండి 85 రెట్లు ఎక్కువ ఉందని అంచనా వేశారు.[11][12]
సంక్రమణకు రోగనిరోధక ప్రతిస్పందనలో భాగంగా IgM, IgG తో ప్రతిరోధకాల ఉత్పత్తి చెందించటం. సంక్రమణ సమయంలో స్థాయిలు బాగా వర్గీకరించబడనప్పటికీ, FDA ప్రకారం, SARS-CoV-2 కు IgM ప్రతిరోధకాలు సాధారణంగా ప్రారంభ సంక్రమణ తర్వాత చాలా రోజుల తరువాత రక్తంలో గుర్తించబడతాయి.[13] సార్స్-సిఓవి-2 కు IgG ప్రతిరోధకాలు సాధారణంగా సంక్రమణ తర్వాత 10-14 రోజుల తరువాత గుర్తించబడతాయి, అయినప్పటికీ అవి ముందుగానే గుర్తించబడవచ్చు. సాధారణంగా సంక్రమణ ప్రారంభమైన 28 రోజుల తరువాత కేసులు గరిష్టంగా ఉంటాయి.[14]
